Kindergarten Enrollment

In order to enroll in kindergarten, your child should be five years of age by September 1. At time of enrollment, we are requesting the following:

• A completed registration form
• Proof of residency
• Proof of age and identity – a copy of one of the following:
  • Certified birth certificate
  • Baptism record
  • A social security card or social security application with registering child’s name
  • A notarized affidavit explaining the lack of birth certificate
